The Pokémon creators and artists work hard to keep similar character designs between the anime, manga, and game. Blue is no exception to this rule. During most of the games Blue's outfit matches (or is at least similar to) of the manga version's outfits or the outfits of his anime counterpart, Gary Oak.
During the 1st and 2nd Generation games Blue has spiky brown hair with bangs that often fall over one eye. His hair color changes to red to match the protagonist’s new lighter hair color. His bangs usually stay towards the right side of his face however depending on which way he's facing the bangs be on the left side. Blue has green eyes. His eye color may seem strange based on his name however, in Japan Blue and Green names are switched, which explains his green eyes. Blue has a slim build with some slight muscle and he holds himself with an air of superiority.
In the original games Red, Green and Blue, Blue’s outfit features a ripped sleeveless shirt (of unknown color) with a circular emblem over his heart. His shirt is tucked into his pants and he wears a belt with a fanny pack over top of them. He has a pair of sneakers to complete his outfit. When you face him as the champion, he changes into a black shirt. Blue wears a jacket with white lining over top. He wears boots with black trim on them. This outfit is highly reminiscent of the manga Blue’s Gold/Silver/Crystal Arc outfit.
In the Pokémon Yellow games Blue appears he has a long sleeved indigo sweater with a rainbow clover emblem on the lower left hand side. He wears black pants, though it is unknown as to whether they’re meant to be slacks, jeans, or sweatpants. Blue’s pants are tucked into his brown boots with black straps around them to hold them up. Blue wears a low hanging yellow and green pendant around his neck. Blue’s outfit remains the same throughout the Gold, Silver, and Crystal games.
During the Fire Red and Leaf Green games Blue’s outfit changes, he trades in his black t-shirt for a black polo shirt with a green sheen to it. He replaces his slacks and boots for a pair of blue cargo pants and black sneakers. Blue shortens the chain that he wore his pendant on in Yellow Version and adds black sweatbands with a blue sheen around his wrists. He also wears a (somehow) stylish gray and black fanny pack around his waist to hold his Pokéballs.
In the Heart Gold and Soul Silver Games Blue’s hair is once again depicted as being brown. His outfit is similar to his champion outfit in the Red, Green, and Blue games. He wears a navy blue jacket with two identical breast pockets. He once again has a fuzzy white color. He wears a white shirt underneath it all. Blue’s pants become khaki cargo pants and he keeps the same black shoes from his Fire Red and Leaf Green outfit.
